Abstract

This study aims to analyze the causes of failure in the bark conveyor motor at Kone 1 WP 9 at PT Indah Kiat Pulp & Paper and to formulate maintenance recommendations to reduce failure frequency and production downtime. A descriptive
case study method was employed, involving data collection through field observations, technician interviews, a review of maintenance history, and motor condition measurements using a megger, thermogun, vibration meter, and stroboscope. The failure data analyzed included instances of bearing wear on October 13, 2025, severe overheating on February 24, 2026, and issues involving a dirty fan and loose terminals
on May 16, 2026. The analysis revealed that the primary causes of failure were a combination of bearing wear inducing vibration, cooling system impairment due to a dirty fan and obstructed airflow, and poor terminal connection quality leading to localized heating and unstable current. These factors were exacerbated by high workloads and suboptimal maintenance practices. Based on these findings, the implementation of preventive and predictive maintenance programs is recommended; these should include an operating-hour-based bearing lubrication schedule, routine cleaning and inspection of the fan and cooling fins, periodic terminal tightening and cleaning, and vibration and temperature monitoring for early detection. Implementing these recommendations is expected to enhance motor reliability, reduce downtime, and ensure the continuity of the production process.
