Abstract
Information technology has now become a necessity for society, including in the health sector. At Dr. Moris Bationg Sihite information technology can be used to register patients online and manage medical record data such as creating web-based service applications. However, the management of this clinical medical records unit still uses a spreadsheet application or still uses paper records. The spreadsheet application used also still has shortcomings because the clinic staff also have to understand the formulas to use the application even though the staff is not an operator. As in the general practice of Dr. Moris Bationg Sihite registration and processing of patient data is still written on paper and recorded for archives, this is the cause of inefficient service at the clinic because data can be lost or damaged. Therefore, this research aims to facilitate the service process in clinics such as registration and processing of patient data in order to reduce the risk of data damage. This application is designed to be web-based using the PHP (Pear Hypertext Preprocessor) programming language and MySQL as the database. In making this application, it is adapted to the needs that have been analyzed in the current clinic service system. It is hoped that this application can facilitate the implementation of activities in clinic services and improve services to the community.
